MDT Meeting: discussion of care plan for Medical Student OSCE

Meeting led by one of the allied health professionals; fill in any gaps in care plan

  1. Introduction

    1. Introduce name and role: Sent by Registrar, Dr Peters, to gather information regarding Mr Jones’ care plan

    2. Apologise for late arrival.

    3. Explain objective: gather information and report back

    4. Ask team members to introduce themselves and their roles

    5. Check discussing correct patient: Mr Jones, 42-year-old man who was admitted to the neurosurgical unit following a severe head injury (head on collision) and has had a prolonged stay in ITU.

    6. Describe state of patient. According to Dr Peters, Mr Jones is medically fit for discharge. Asks opinion of other healthcare workers: confirm pt is stable for discharge

  2. To nurse – how is patient generally?

    1. Can pt get out of bed? Wash and feed unaided? Go to toilet unaided? Walk unaided?

    2. Asks about patient’s mood

    3. Pt sleeping well?

  3. To physio – current function

    1. How is walking?

    2. Weakness present? (E.g. in stroke) – will this improve?

    3. If necessary, can physio do home visits?

    4. Stairs assessment done?

  4. To OT – physical home situation

    1. Seen patient’s home? Is it in a state that pt could return to? Kitchen and bathroom tailored to needs?

    2. If physio says stairs are a problem – stair lift needed: when can OT have this done?

  5. Social worker – care at home

    1. Spoken to family?

    2. Family situation and health status?

    3. Who will care for patient? Can home carer be helped with anything?

  6. Speech therapist

  7. Dietician

  8. Rounding up

    1. Suggests and gains agreement on discharge date

    2. Checks for further issues/info; summarises

    3. Suggest discharge plan; check arrangements for follow-up appointments

    4. Confirm that all agree

  9. Other marks: knowledge of roles of team members; respect for each member’s opinion; gives everyone opportunity to contribute

  10. Last 2 minutes: summary to examiner:

    1. Summary of medical status

    2. Opinions and concerns of health professionals

    3. Mention allied health professionals are happy for discharge as long as home visit is ok
