Meeting led by one of the allied health professionals; fill in any gaps in care plan

Introduce name and role: Sent by Registrar, Dr Peters, to gather information regarding Mr Jones’ care plan
-
Apologise for late arrival.

Explain objective: gather information and report back

Ask team members to introduce themselves and their roles

Check discussing correct patient: Mr Jones, 42-year-old man who was admitted to the neurosurgical unit following a severe head injury (head on collision) and has had a prolonged stay in ITU.

Describe state of patient. According to Dr Peters, Mr Jones is medically fit for discharge. Asks opinion of other healthcare workers: confirm pt is stable for discharge

Can pt get out of bed? Wash and feed unaided? Go to toilet unaided? Walk unaided?

Asks about patient’s mood

If physio says stairs are a problem – stair lift needed: when can OT have this done?

Who will care for patient? Can home carer be helped with anything?
